Release of Liability, Waiver of Claims, Assumption of Risks and Indemnity Agreement for all Attendees

I understand that this agreement affects my legal rights and the legal rights of the minors and that by accepting the consent below I acknowledge that I have read and understood the disclosure of risks, voluntarily accept those risks, and agree to be bound by all terms and conditions of this agreement.

Masjid Toronto reserves the rights to edit or change the rules of the agreement at any time without prior notice.

Liability: the participator shall protect, reimburse, and hold harmless MAC Masjid Toronto from and against any and all claims and expenses in connection with:

1. Any loss or damage to person or property caused by the use of the premises by the child(ren) signed up to participate in MAC Masjid Toronto Summer Camp; and,

2. MAC Masjid Toronto shall not be liable or in any way responsible to the child(ren) in respect of any injury, loss, or damage to the child(ren) or property occurring on the premises to the child(ren) or any other person.

3. The parent(s) of the child(ren) agrees not to do or allow to be done, upon the premises anything which may make void or voidable any insurance on the building against fire or other risk or casualty usually insured against or which may cause any additional or increased premium to be payable for such insurance. If additional or increased premiums become payable because the child(ren) failed to comply with this subsection the parent(s) will pay the Masjid Toronto the amount of such increase in insurance premiums.

The parent(s) coming to drop-off/pick-up their child(ren) any class is not to leave any children related to them unattended; MAC Masjid Toronto is not liable to any injuries to the child that should be under the supervision of the parent(s) while at the center. No children should be dropped off at MAC Masjid Toronto without any supervision.

As a parent, I understand that my child(ren) is to adhere to the Islamic code of clothing at all times while on Masjid Toronto property; it is mandatory to wear clothing that covers the appropriate neck area, arms, and legs for both boys and girls.

I consent for pictures/videos to be taken of my child(ren) during the classes and field trips for social media and promotional purposes.

The parent(s) is responsible for paying any damages done by their child(ren) during or after the program.

I agree to be mindful of Islamic etiquettes when dealing with Masjid Toronto staff and volunteers.

Please do not register your child if they require extra attention or special needs. MAC Masjid Toronto does not yet have the professional help needed to take care of your child.

I understand that the fee is non-refundable once the classes begin.
