Release of Liability, Waiver of Claims, Assumption of Risks and Indemnity Agreement for all Attendees
I understand that this agreement affects my legal rights and the legal rights of the minors and that by accepting the consent below I acknowledge that I have read and understood the disclosure of risks, voluntarily accept those risks, and agree to be bound by all terms and conditions of this agreement.

ICCO reserves the rights to edit or change the rules of the agreement at any time without prior notice.
Liability: the participator shall protect, reimburse, and hold harmless the Islamic Community Centre of Ontario (ICCO) from and against any and all claims and expenses in connection with:
1. Any loss or damage to person or property caused by the use of the premises by the participator or any third party allowed to enter by the visitor; and,
2. The ICCO shall not be liable or in any way responsible to the participator in respect of any injury, loss, or damage to persons or property occurring on the premises to the visitor or any other person.
3. The participator agrees not to do or allow to be done, upon the premises anything which may make void or voidable any insurance on the building against fire or other risk or casualty usually insured against or which may cause any additional or increased premium to be payable for such insurance. If additional or increased premiums become payable because the participator failed to comply with this subsection the participator will pay the ICCO the amount of such increase in insurance premiums.
Supervision of all children and children-related activities is required in all areas occupied and accessible to the participator; the provision of such supervision is a condition of granting permission to use the center.
The participator is not to leave any children related to them unattended; ICCO is not liable to any injuries to the child that should be under the supervision of the participator while at the center. No children should be dropped off at ICCO without any supervision.
Tables, chairs, equipment, and other furniture shall not be moved from one area of the center to another.
The participator is expected to exercise care of the centre and its equipment. The visitor is expected to keep the center clean and tidy. The visitor is expected to pick up any food on the floor and throw it in the nearest garbage bin.
No soccer is allowed in the gym.
All activities MUST stop by Athan.
Music is not allowed inside or outside the premises of ICCO.
Adhere to the Islamic code of clothing at all times while on ICCO property; it is mandatory to wear clothing that covers the appropriate neck area and legs.
I consent for pictures/videos to be taken of my child during the field trip for social media and promotional purposes.
The individual and/or the organizer is responsible for paying any damages and extra cleaning charge that is done during their program and/or event and/or activities at ICCO.
I agree to be mindful of Islamic etiquettes when dealing with ICCO staff and volunteers.
As a parent I am responsible for registering my child in the age-appropriate class and correct gender, I understand that failure to register my child according to ICCO guidelines will result in my child’s dismissal.
Please do not register your child if they require extra attention or special needs. ICCO is does not yet have the professional help needed to take care of your child.
I understand that there is a $10 non-refundable registration fee per child included in the price.
I understand there is extra charges for the uniform and the belt; and I agree to pay the charge should I choose to register my child in this program.
I understand that ICCO is not responsible for any injury that may occur to my child during Taekwondo Classes as this is a sports class and injuries are inevitable.
I understand that there is no refund once the classes have begun. I understand that if I wish to have my child discontinue the program I must inform the ICCO office via email, four weeks in advance.
