Release of Liability, Waiver of Claims, Assumption of Risks and Indemnity Agreement for all Attendees
I understand that this agreement affects my legal rights and the legal rights of the minors and that by accepting the consent below I acknowledge that I have read and understood the disclosure of risks, voluntarily accept those risks, and agree to be bound by all terms and conditions of this agreement.

ICCO reserves the rights to edit or change the rules of the agreement at any time without prior notice.
Liability: the participator shall protect, reimburse, and hold harmless the Islamic Community Centre of Ontario (ICCO) from and against any and all claims and expenses in connection with:
1. Any loss or damage to person or property caused by the use of the premises by the participator or any third party allowed to enter by the visitor; and,
2. The ICCO shall not be liable or in any way responsible to the participator in respect of any injury, loss, or damage to persons or property occurring on the premises to the visitor or any other person.
3. The participator agrees not to do or allow to be done, upon the premises anything which may make void or voidable any insurance on the building against fire or other risk or casualty usually insured against or which may cause any additional or increased premium to be payable for such insurance. If additional or increased premiums become payable because the participator failed to comply with this subsection the participator will pay the ICCO the amount of such increase in insurance premiums.
Supervision of all children and children-related activities is required in all areas occupied and accessible to the participator; the provision of such supervision is a condition of granting permission to use the center.
The participator is not to leave any children related to them unattended; ICCO is not liable to any injuries to the child that should be under the supervision of the participator while at the center. No children should be dropped off at ICCO without any supervision.
Tables, chairs, equipment, and other furniture shall not be moved from one area of the center to another.
The participator is expected to exercise care of the centre and its equipment. The visitor is expected to keep the center clean and tidy. The visitor is expected to pick up any food on the floor and throw it in the nearest garbage bin.
No soccer is allowed in the gym.
All activities MUST stop by Athan.
Music is not allowed inside or outside the premises of ICCO.
Adhere to the Islamic code of clothing at all times while on ICCO property; it is mandatory to wear clothing that covers the appropriate neck area and legs.
I consent for pictures/videos to be taken of my child during the field trip for social media and promotional purposes.
The individual and/or the organizer is responsible for paying any damages and extra cleaning charge that is done during their program and/or event and/or activities at ICCO.
I agree to be mindful of Islamic etiquettes when dealing with ICCO staff and volunteers.
As a parent I am responsible for registering my child in the age-appropriate class and correct gender, I understand that failure to register my child according to ICCO guidelines will result in my child’s dismissal.
Please do not register your child if they require extra attention or special needs. ICCO is does not yet have the professional help needed to take care of your child.
I understand that there is a $10 non-refundable registration fee per child included in the price.
In consideration of Mini MAC Muslim Early Years Centre, at 2550 Dunwin Dr, Mississauga, ON L5L 1T1 (Islamic Community Centre of Ontario), granting any and all children who enter Mini Mac’s facility under the direct or indirect supervision of the undersigned, to use the Facility, I, hereby on behalf of myself, my Child/Children, my Children’s heirs, executors, administrators, or anyone who might claim on their behalf warrant and agree as follows:
1. I represent and warrant that I am the parent or legal guardian of the below named Child/Children. I will be responsible for the supervision of the Child/Children.
2. By entering the premises, I acknowledge and I am fully aware of the inherent risks present in an indoor play environment. That the equipment and the nature of its use can be dangerous and may result in serious injury to myself and/or my Child/Children. These risks include, without limitation, damage to personal property, personal injury or death.
3. I hereby knowingly and voluntarily assume and accept all risks, including all presently unknown or unforeseeable risks, and assume all responsibility of risks to myself, my Child/Children, my personal property and the personal property of my Child/Children upon signing this Waiver.
4. I warrant and represent that I have read the Mini MAC Play Rules, which form part of this Waiver. I agree to, and shall ensure my Child/Children use the facility only in accordance with the Rules. I acknowledge that failure to abide by the Rules or to use the equipment in a safe and appropriate manner may, at the sole discretion of Mini MAC, result in my or my Child’s/Children’s removal from the Facility.
5. I understand and agree that Mini MAC, its shareholders, officers, directors, employees, management, contractors, agents or other representatives, shall not be liable for any injury, damage or loss that I or my Child/Children may suffer as a result of using the Facility. I understand and agree that Mini MAC and its Representatives are not responsible for the supervision of my Child/Children and shall not be held liable for any reason whatsoever for failing to supervise my Child/Children, including child disappearance.
6. I hereby release, waive and forever discharge Mini MAC and its Representatives from any and all liability, claims, demands, losses, or damages, however caused, whether related to property damage, theft, and/or personal injury (including death), and whether based on tort, intentional act, strict liability, negligence, and/or negligent rescue, that I or my Child/Children may suffer as a result of my Child/Children using the Facility, including as a result of the negligence of Mini MAC or its Representatives. I will indemnify, save and hold harmless each of Mini MAC and its Representatives from any and all claims, expenses, legal fees, losses, liabilities, damages, or costs which relate to, or arises from, this Waiver, to the fullest extent permitted by law.
7. I represent and warrant that my Child/Children is/are old enough to participate.
8. I agree and acknowledge that my execution of this Waiver on the initial visit, or for one of my Children, will and does authorize Mini MAC to enter this Waiver into its database and use it as a continuous, multi-use waiver for my Childs/Childrens ongoing entry to play in the Facility or use it as a Waiver executed for my other Child/Children. I hereby expressly authorize Mini MAC to use this Waiver as a multi-use wavier until such time as I revoke it in writing.
9. I agree and acknowledge that Mini MAC shall use reasonable efforts to ensure that the Facility is a nut free facility, however due to numerous factors outside its control, Mini MAC makes no guarantee, warranty or representation that the Facility is a nut free facility. I accept any and all risk that this may cause for my Child/Children.
Mini MAC Play Rules
1. All people must be supervised by a responsible adult or caregiver at all times. A responsible caregiver is someone who is a minimum of 16 years old.
2. For the safety of our youngest Participants, walking is always encouraged.
3. All toys must remain in their place to allow all Participants an equal opportunity to play. Please ensure that your child leaves toys in appropriate places.
4. Mini MAC is not responsible for any lost or stolen items.
5. Have Fun and Play Safe.
