1. Medical:
I authorize the Villeray Community Center to make the necessary arrangements regarding the health of the participant in the event of injury or illness.
(Parent/guardian if participant is under 18)
2. Promotion:
Authorization to take a photo or video of the participant for promotional purposes for the Villeray Community Center.
3. This contract is personal; it cannot be assigned or transferred to another person; Failure to comply with this clause results in suspension of classes and registration. No reimbursement may be required.
4. Full registration fees (and other related costs) must be paid at the time of registration; the participant will not be able to attend the courses without paying the fees.
5. A participant who has not paid the fees for past sessions will not be able to renew their registration.
6. The participant undertakes to respect the safety rules in force and to be polite towards teachers, supervisors, volunteers and other participants; Failure to comply with this clause may result in expulsion of the participant. No reimbursement may be required.
7. Cancellation of the subscription, in the first month of registration, results in a full refund of the other months. Otherwise, three months will be charged to the participant and the remainder of the subscription will be refunded.
8. The Villeray Community Center disclaims all responsibilities with regard to the participant's possessions, even those left in class.
9. After 15 minutes of the end of each class, a fee of one dollar will be applied for each minute late. This amount must be paid on site to the daycare manager.
10. Quran lessons:
The duration of a lesson is 3 hours, and any delay directly affects the student's performance. The latecomer will have a late ticket to be able to return to class.
11. Parents/guardians are responsible for IMMEDIATELY taking and picking up their children from the classroom at the beginning and end of each lesson. The Villeray Community Center is not responsible for children outside the classroom and outside the hours scheduled for their lessons.
12. The Villeray Community Center reserves the right to modify schedules without notice. Parents are responsible for checking their email account and consulting the ccvmac.ca website.
